Donations to offset the intake cost to Faith N Friends are greatly appreciated. They are, however, not required as a condition of intake.

If you are unable to transport the horse(s), there is a fee of $2.00 per mile to offset the cost of transportation. Yes, I understand.

Representations and Warranties
I hereby represent and warrant to Faith N Friends that the undersigned is the sole owner of the above-described equine; has the authority to surrender this equine for adoption/placement or sanctuary at Faith N Friends, that no other person or person’s signature is required to initiate a valid transfer of ownership and registration papers on this equine (if available); and that no other person or persons has any legal equitable ownership interest in this equine.
Surrender Rights
By executing this document I understand that I am giving up forever all rights to, titles to, and interests, regardless of the delivery or non‐delivery of registration papers, to the above equine voluntarily and without coercion or threats of any kind. I further understand that all future decisions regarding the placement of this equine will solely be those of Faith N Friends. I also understand if this equine is found to be unsuitable for placement for reasons determined by Faith N Friends and/or a licensed veterinarian that the equine may either be retained as a Sanctuary Ambassador at Faith N Friends or euthanized. Lastly, it is understood that the surrendering party shall hold Faith N Friends and all its officers, directors, employees, and volunteers harmless from any claims of damage, injury, or acts of negligence arising from this surrender.
This is a legally binding document for the irrevocable surrender of an equine for placement in an adoptive home by Faith N Friends. Please read this document carefully and thoroughly before signing. Take as much time as you need to consider this decision.

SEVERABILITY:: In the event that a court of competent jurisdiction finds any term or clause in this Agreement to be invalid, unenforceable, or illegal, the same will not have an impact on other terms or clauses in the Agreement or the entire Agreement. However, such a term or clause may be revised to the extent required according to the opinion of the court to render the Agreement enforceable or valid, and the rights and responsibilities of the parties shall be interpreted and enforced accordingly, so as to preserve their agreement and intent to the fullest possible extent.

**Adopters must be legal adults. The age requirement to sign a contract is 18 years of age, and applications/contracts signed by minors will be deemed invalid contracts. A child under the age of 18 is considered a minor and is unable to sign a contract.
